Personal Health Record (PHR)

A Personal Health Record (PHR) is a tool that enables you to collect, manage and share past and current information about your health or the health of someone in your care. A PHR can give medical care providers insight into your personal health story.

Things to know about a Personal Health Record:

  • A PHR should enable access to all your important health information.
  • Information in your PHR should be accurate and complete.
  • You should have control over how your health information is accessed, used, and disclosed.
  • A PHR does not replace medical records of healthcare services. 
  • Medical records and your PHR are not the same. Medical records contain information about your health compiled and maintained by each of your healthcare providers. A PHR is information about your health compiled and maintained by you.

What to include:

  • Personal information including name, address and birth date
  • People to contact in case of emergency 
  • Primary care physician and other medical professionals that have treated you
  • Names, addresses, and phone numbers of your physician(s), dentist and specialists 
  • Health insurance information 
  • Living wills, advance directives, or medical power of attorney 
  • Organ donor authorization 
  • A list and dates of conditions and surgical procedures
  • Current medications and dosages 
  • Immunizations received and their dates 
  • Allergies or sensitivities to drugs, foods, other materials
  • Important events, dates, family health history and hereditary conditions
  • Results from a recent physical examination or ongoing progress notes
  • Opinions of specialists
  • Important tests results; eye and dental records 
  • Correspondence between you and your healthcare and insurance provider(s) 
  • Any information you want to include about your health – such as your exercise regimen, blood glucose testing, blood pressure, herbal medications you take and more.
